The Alexian Brothers understand the importance of home in the lives of aging adults. All the clichés apply. Home is where our hearts are. Home is sweet. Sure travel is exciting, but home is where we want to live out our lives.
The Alexian Brothers in the Chattanooga area have put together a program that helps older adults remain in their own homes despite advancing years, changing health and declining physical ability. Called Live At Home, this unique program takes all the Alexian Brothers have learned about caring for the aging and elderly and combines it to meet the total needs of older adults in the place they love most . . . their own homes.
Designed for adults age 55 and over who want to plan ahead for their long-term needs, Live At Home is a continuing care retirement option that is practical and affordable.
Four Levels of Service
Those who enroll in Live At Home secure the benefits of escalating levels of care designed to provide essential health and personal services when and where needed.
Through each level of care (described below), Live At Home members are guided by a professional service coordinator, who becomes a consultant, confidant and friend as the years go by. The role of the service coordinator is to respond to changing member needs and to organize resources to help members remain safe, secure and living at home.
Level One – Wellness By Design
Designed to meet the needs of members when they are healthy and active, features include:
An emphasis on improving wellness and health.
Includes a baseline health assessment.
Counseling and education on nutrition, exercise and medical treatments.
Referrals to trustworthy services for home maintenance and repair.
Level Two – Intervention By Design
Designed to respond to changing needs, services at this level include:
All services of level one.
Increased support from the services coordinator and staff in response to specific needs.
Support with normal tasks of daily living, such as driving and food preparation.
Assistance with light home chores.
Level Three – Support By Design
A more intensive level of service to designed to respond to declining physical ability.
All services of level one and two.
Intensive care management from the Live At Home staff.
In-home assistance with meals, bathing, grooming and homemaking.
Level Four – Care By Design
Designed to provide nursing home level care, but for most people, without the nursing home.
Intensive, daily support and services in the home and in Alexian facilities.
Transportation.
Enrollment in Program of All-Inclusive Care of the Elderly (PACE), at no additional charge.
Prescription medications at no additional charge, if PACE option is chosen.
Medical care, hospital care and rehabilitation without copayment, if PACE option is chosen.
Option to move to Alexian facilities.
