Trinity Hospice Care is a Promise... 

A promise given to those patients facing a terminal illness when choices seem so few. Hospice is about living life to the fullest in secure and familiar surroundings with those who matter most - family and friends. Trinity Hospice offers terminally ill patients an alternative to routine home care and repeated hospitalizations.

 

Trinity Hospice proudly offers comprehensive care focused on easing the physical, emotional and spiritual pain that often accompanies terminal illness.  Offering outstanding care, the Trinity Hospice Team members are dedicated professionals and trained volunteers who specialize in meeting the individualized needs of terminally ill patients and their families.

 

Care Includes:

  • Hospice physician and patient's regular physician
  • Regularly scheduled nursing visits
  • Nurse on-call twenty-four hours a day, seven days a week, 365 days a year
  • Home Health Aids provide personal care and assistance with the activities of daily living
  • Counselors assist families to identify their personal and community resources and develop support networks
  • Education about the illness and  what to expect as the disease progresses and how to cope
  • Medical equipment, supplies and medications are coordinated by the physician and the hospice nurse
  • Hospital inpatient care and family respite when medically necessary
  • Spiritual and bereavement  support provided  for loved ones after their loss
  • Volunteer support and assistance for the patient and caregivers

Who is Eligible for Hospice Care

Trinity Hospice offers services to patients who have a terminal illness where the goal is pain management and symptom control.  Financial coverage is available through the Hospice Medicare Benefit and many private insurance companies.  No person will be denied services based on age, race, national origin, disease, handicap, religion, gender, color, sexual orientation or inability to pay.
